CCDD Community Survey

We need your help. The CCDD is very interested in how you feel about the COVID 19 vaccines. We need to gather your input in order to support Kentuckians with disabilities. Please take a few moments to complete our survey. All responses are anonymous. 

The vaccines are being given now in Phase 1 of Kentucky's vaccine plan.
